Impaction Masterclass: Diagnosis, Timing, Outcomes & Pitfalls

Impacted teeth are among the most challenging problems in orthodontics, yet clinical attention often focuses mainly on maxillary canines. This masterclass will address canines but broaden the perspective, providing an evidence-based framework for also managing incisors and molars. Although tooth type varies, the determinants of success remain the same and follow the “Impaction Equation”: accurate 3D localization combined with optimal timing of intervention. Using clinical case studies, from predictable successes to complex failures, I will highlight the most frequent and costly pitfalls in diagnosis, surgical exposure, anchorage planning, and mechanotherapy.

Participants will learn how to:

·      Recognize the dental and skeletal effects of a severe impaction

·      Decide on the ideal timing for initiating the orthodontic eruption

  • Plan the most effective mechanotherapy for a successful treatment outcome

Dr. Stella Chaushu is Vice Dean, Full Professor and Chair of Department of Orthodontics, Hebrew University-Hadassah School of Dental Medicine, Jerusalem, from where she earned her DMD and Specialist in Orthodontics degrees and her PhD in Immunology.  She is extensively involved in promoting orthodontic research through her numerous leadership positions, scholarly publications, and conference presentations, which have garnered her global recognition, including the 2025 WFO Outstanding Educator award. She has authored more than 120 publications, 20 chapters and two patents. Her work has led to transformative advancements in the management of impacted teeth, Special Need patients and the role of innate immunity in orthodontics.

Dr. Chaushu (or an immediate family member) is not associated with, employed by, nor consults for any manufacturer, supplier, or retailer of dental instruments or materials.

Struggling with Impacted Canines?

Management of Impacted Canines with a Straight Wire Approach using Passive Self-Ligation Mechanics

Orthodontic management of impacted canines is often reported by clinicians as one of the most challenging aspects of orthodontic treatment.  It is commonplace to use adjunctive appliances and heavy base wires to provide anchorage to assist in their eruption.  What many orthodontists often miss is that the surgical procedure is the first key to success when managing impacted canines.  To this end, the course will review how the orthodontist can work with their surgeons to ensure the surgical procedure provides them with the best chance of success.  The second key is the use of passive self-ligation mechanics.  With thorough review of mechanical systems, attendees will learn how to easily and consistently bring in impacted canines.

Course Objectives:

  • Learn the importance of collaboration between the orthodontist and surgeon when managing impacted teeth.
  • Understand how specific surgical techniques can improve outcomes for orthodontic eruption of impacted teeth.
  • Learn how the use of passive self-ligation can virtually eliminate the use of adjunctive appliances often required to orthodontically erupt impacted teeth.
  • Understand why straight wire mechanics without the use of base wires is sufficient to efficiently and effectively erupt impacted teeth when using passive self-ligation.

Dr. Kozlowski graduated UB Dental in 1996, UB Ortho in 1998, and has been in private practice in Southeastern CT since 1998.  He has lectured extensively both in the US and internationally on passive self-ligation mechanics, facial driven treatment planning, digital orthodontics, early treatment, management of impacted canines and the business of orthodontics.  He is the Co-Founder of OrthoFi. His wife Amy Napierala is a pediatric dentist (UB dental 1996), daughter Amelia is currently a D3 at UB Dental and son Jake is a senior lacrosse goalie at Colgate University and currently applying to dental school (hopefully a future UB Dental grad!)

Dr. Kozlowski (or an immediate family member) has a significant relationship with the following commercial organizations whose products he will refer to in his presentation: Ormco Corp. and OrthoFi.
